Health, medical, and other required forms must be submitted via the Magnus Health system.
The medical forms and documents in the system are required by the state of Kentucky and Kentucky Country Day School. These forms must be completed by August 1, 2024.
All students are required to be up to date on all vaccines.
These vaccines need to be printed on the Kentucky Immunization Certificate and uploaded to Magnus.
Please note that all new students and all sixth graders are required to have a completed health physical on file. This is not the same as a sports physical.

KCD uses Magnus Health SMR (Student Medical Record), a HIPAA-compliant, online database to collect student medical information and other important documents for the school. This confidential web-based system will provide our parents and appropriate employees at KCD with access to your child’s health record as well as the ability to make updates when needed. Additionally, parents will have less paperwork to complete each year as the system will maintain the information for the duration of your child’s education at KCD and be accessible after graduation.

- We highly recommend that you take the time to watch the tutorial on the Magnus website before you begin!
- When logging in for the first time, please reserve 20 to 30 minutes per child, depending on particular circumstances. Returning families will only need to review and update the information.
- While entering this data it will be helpful to have your pediatrician’s address, telephone number, and your insurance card available.
- You will receive weekly email reminders as long as there are outstanding items in your account.
- When your student comes to the nurse’s office for treatment, you will receive notification via email to login to Magnus. Once logged in, you are able to see what the reason for the student’s visit was and what was done to help your student. All information is available on Magnus to view at any time.

- Log in to your KCD account using the MyKCD link above.
- Click the Resources tab.
