All Students

The following documents and forms are required for all students. These records must be on file prior to the opening of school or the school reserves the right to refuse attendance.

  1. Kentucky state law mandates that all students have a current Kentucky immunization certificate and a physical examination form on file.
  2. In addition, parents must submit an emergency health information form and medical information form for each student. These documents are attached below.
  3. In addition, any child six years old or younger must have a vision examination by an optometrist or ophthalmologist. The Kentucky Eye Examination Form for School Entry is available from the eye doctor’s office.
  4. To abide by Kentucky state law, we are required to have a certified copy of your child’s birth certificate (not the hospital birth certificate) for every student’s permanent record folder. Records for students born in Kentucky can be ordered through the Department of Vital Statistics. Certified birth certificates cost $10 and can take up to thirty days to arrive. Credit card orders are assessed an additional $10.50 charge. Additional information can be found at http://chfs.ky.gov/dph/vital/.

KCD expects full disclosure of relevant health and medical information. The school will work to inform faculty and staff of medical conditions as necessary.

New students

All students new to Kentucky Country Day School must have had a physical within twelve months of the start of school. These records are to be on file as of August 15. All medical records are to be up to date prior to our opening day or the school reserves the right to refuse attendance. The completed examination form and immunization certificate signed by the doctor should be mailed to KCD to be filed in the appropriate division office. The immunization record must be on a Kentucky form. Out of state certificates will not be accepted. When returning forms, please indicate on the envelope which grade your child is entering.

Physical exam forms and immunization certificates can be provided by your doctor’s office.

Returning students

With the exception of sixth-graders and students wishing to participate in athletics, a physical exam form is not required each year for returning students.
