UPPER SCHOOL BACK TO SCHOOL INFORMATION
The forms listed below contain information specific to students in the Upper School (grades 9–12). Forms and information relevant to students in all divisions can be found on the Back to School home page.

The following forms must be turned in by August 17, 2008

Parking Registration (if you drive to school)
Honor Pledge (for freshmen and new to US)
Assumption of Risk (all US students)

Absence & Attendance Policies
This document explains absence and attendance policies in the Upper School.

Non-Medical Excused Absence Form
This form is used to request a non-medical excused absence.

College Visit Form
This form is used to request an excused absence for a college visit.

Upper School Dress Code Guidelines
This document explains the dress code guidelines for grades 5–12.

Upper School Honor Pledge
This document explains the upper school Honor Pledge and must be signed by every student in the Upper School.

Parking Registration Form
This document must be completed by the parent of every student who wishes to drive to school.

Assumption of Risk
This document must be completed before students can participate in KCD's ropes course.

Physical exam form for upper school student-athletes
This document must be submitted by any student wishing to participate in athletics at KCD.

Immunization Certificate
All new students must have an updated immunization certificate on file before the start of school. Your doctor can provide you with this certificate.

Summer Reading
As the Upper School Reading List explains, juniors and seniors must make one of their summer reading selections based on their course electives for next year. These course-specific reading assignments are posted here.
