The forms listed below contain information specific to students in the Middle School (grades 5–8). Forms and information relevant to students in all divisions can be found on the Back to School home page.

Middle School Supply Lists
Note: The ordering deadline and account instructions in the attachment were updated on June 25. This document lists the supplies required at each grade level in the Middle School. As a convenience, the Parent Association will be taking supply orders. Orders are due by July 10 and will be delivered directly to your home in late July/early August. Ordering instructions are posted here.

Middle School Dress Code Guidelines
This document explains the dress code guidelines for grades 5–12.

PE Clothing Guidelines for grades 5–8
This document explains the gym clothing requirements for grades 5–8.

Gym Clothing Ordering Procedures for grades 5–8
Parents who wish to do so may order KCD gym clothing online through the KCD Alumni Association. This document explains the ordering procedure. Clothing purchased through the Alumni Association meets all requirements of the PE department. Please review this document before placing your order.

Medical form for all sixth-graders
This form must be completed by all sixth-graders at KCD, whether new or returning. This form is also available at your doctor's office.

Physical exam form for middle school student-athletes
This form must be completed by middle school students wishing to participate in athletics. This form is required by the KHSAA and must be completed in addition to any other medical forms required by KCD.

Immunization Certificate
All incoming sixth-graders and all new students must have an updated immunization certificate on file before the start of school. Your doctor can provide you with this certificate.

BACK TO SCHOOL POOL PARTY
The Middle School Pool Party will be held from 2:30–4:00 p.m. on Wednesday, August 20, in the Commons Room and pool area. Bring the whole family and join us for swimming and snacks.

Schedule for Middle School Pool Party:

2–3:00: Middle school advisors will be in their classrooms, so please stop by to meet your advisors and other classmates. Your advisor will be happy to answer your questions about the first day of school.

2:30: Swimming pool opens. Lifeguards will be on duty at the pool.

2:30–4:00: Snacks served in the Commons.

3:00: Middle school classrooms close, and teachers will join the picnic.

4:00: Pool closes.
